 A:   A referral dated within the past 12 months is required to begin services. Please have your child’s pediatrician or medical specialist fax the referral to our office (702-920-8257). Once received, our team will review the information, confirm that everything is complete, and reach out to discuss your availability. If we have an opening that fits, we’ll offer those times. If not, we’ll place your child on our waitlist and follow up as soon as a spot becomes available.

 A:  To ensure we’re fully prepared for your child’s initial evaluation, we ask that all comprehensive intake forms be completed at least 24 hours in advance. These forms help us better understand your child’s developmental history and current needs.


We also welcome any relevant documents that can provide additional insight, such as previous evaluations (Medical, Neuropsychological, Educational, Speech & Language, Occupational, or Physical Therapy). If your child has an IEP through their school, sharing that information is also beneficial in creating a more complete clinical picture.

 A: Motivated Kids Therapy is an in-network provider with Nevada Medicaid Fee-for-Service (FFS), Molina Nevada Medicaid, United Healthcare, Aetna, and Tricare. For all other insurance plans, we are considered an out-of-network provider.


Please note that we do not bill out-of-network insurance plans directly. If your plan offers out-of-network benefits, you may be eligible for partial reimbursement by submitting claims independently.


We recommend contacting your insurance provider’s member services (listed on the back of your insurance card) to inquire about out-of-network benefits. Be sure to ask about your deductible, copayments, visit limits, and any specific requirements for submitting claims for reimbursement.

A:  As a courtesy, our team will contact your insurance provider to verify coverage details for therapy services. Once we receive this information, we will reach out to review the benefits and any requirements, including whether prior authorization is needed.


We also strongly encourage you to contact your insurance provider directly to confirm in-network status and verify your specific benefits and coverage for the services being requested. This helps ensure clarity and avoid any unexpected costs.
