A: We will ask you to fill out comprehensive intake forms to provide us with additional information on your child’s development and current functioning. In addition to this any Medical, Neuropsychological, Educational, Speech & Language, Occupational, or Physical Therapy evaluations that have been completed will assist us in developing a more complete clinical picture. If your child is on an IEP at school, this information can be helpful as well.
A: Motivated Kids Therapy is a participating “In-Network” provider within Nevada Medicaid FFS, Molina NV Medicaid, United Healthcare, Aetna, & Tricare networks. We are an “Out of Network” provider for all other insurance plans. In order for services to be billable to an insurance plan other than those we have listed your plan must have “Out of Network” benefits. Unfortunately we do not bill out of network health insurances. We recommend you contact the member services number shown on your insurance card and ask if you have Out of Network benefits. They should be able to give you information as to your deductible, copays and visit limits as well as any restrictions your policy may have and how to submit your invoices for reimbursement.
A: We will contact your insurance company and determine the specifics of your coverage. Once this is completed we will contact you to go over the information we obtained from your insurance company. If a referral or prior authorization is required we will go over that with you as well. We also recommend you contact your insurance provider as well to verify provider in network coverage and benefits/coverage of services being requested.
